- The professional, in-office rate each payer published for your NPI
- Which network the rate belongs to — an HMO and a PPO product can differ
- Rates marked “derived” in the file, clearly flagged as estimates
- Exactly which payers don’t list you at all
It doesn’t
- Invent a national benchmark or a “market rate”
- Cover Medicare or Medicaid schedules — those are public elsewhere
- Show facility or institutional rates
